8. A Health Board Report (Board Report) from a fully equipped hospital, specifically stating "…….(Name of branch)………can conduct specialty training in the Department."

9. Biometric Photographs (4 copies)

10. For those who have previously worked in public institutions, a Service Certificate from the institution where they last worked.

11. Copy of a certified identity card (e-government printout)

12. Residence Certificate (e-government printout)
